We, ALBERTO A. PESIGAN, married, and GLORIA N. LIZARDO, widow, both of legal
age, Filipino citizens and residents of Mabini Street, Poblacion, Tanauan,
Batangas after having been duly sworn to in accordance with law freely depose
and say:
That
we know personally the person of RAFAEL S. ESTEBAN, born of parents Felizardo
Esteban Sr. and Milagros L. Salazar, who is a resident of Mabini Street,
Poblacion, Tanauan, Batangas because we are neighbors since the afore-named
person was still a small boy;
That
the name he has been using and the name by which he is known in the locality is
RAFAEL S. ESTEBAN instead of RAPHAEL S. ESTEBAN, since his schooling and in all
his private and public transaction;
That
the name RAFAEL S. ESTEBAN and RAPHAEL S. ESTEBAN is one and the same person
for all legal intents and purpose;
That
in view of the prevailing discrepancy, we were constrained to execute this
affidavit to attest to the truth or veracity of the facts herein above-stated;
That this
affidavit is executed without the taint of any undue or improper pressure and
influence exerted upon our mind and body.
