Affidavit of Loss for Driver’s License
What is an Affidavit of Loss?
An Affidavit of Loss is a sworn legal document executed by a person who has lost an important item, document, or identification card such as a driver’s license, passport, or ATM card. It serves as a formal declaration under oath that the loss occurred unintentionally and that all efforts to locate the missing item have been exhausted.
This affidavit is often required by government agencies or private institutions before issuing a replacement of the lost document. For instance, the Land Transportation Office (LTO) in the Philippines requires a driver to submit an Affidavit of Loss before a replacement driver’s license can be processed.
The affidavit should be signed by the affiant (the person making the statement) and notarized by a lawyer to make it legally binding. Below is a sample template that you can use and edit based on your personal details and situation.

- I am a duly licensed driver in accordance with pertinent Land Transportation laws, rules and regulations, and was issued a corresponding (Non) Professional Driver’s License with number ____________, which is valid until ____________;
- Sometime on ____________, said driver’s license was misplaced and got lost;
- Efforts were exerted to locate said driver’s license, but in spite of diligent search, it could not be found and the same is now beyond recovery;
- Said Driver’s License has not been confiscated by the LTO, Police, or other Traffic Enforcers for any traffic violation;
- As such, I am executing this Affidavit of Loss to attest to the truth of the foregoing and to support my application for the issuance of a new (Non) Professional Driver’s License, in lieu of the one that was lost.
