single, Filipino and with residence address at J.P. Rizal St., Talisay,
Batangas after having been duly sworn to in accordance with law, hereby depose
and state:
- That I am the owner of RAFJANZ COMPUTER SERVICES with business address at W. Trinidad St., Talisay, Batangas;
- That the said business was registered under the Department of Trade Industry at Lipa City on December 31, 2013 and valid until December 31, 2018;
- That on December 2013 the said business is not operating;
- That for this reason there was no income from the said business for the month;
- That I am executing this affidavit to attest to the truth of the foregoing statements to inform the proper authorities concerned, and for whatever legal purpose this may serve.
