PASSPORT REQUIREMENTS
(BRING ORIGINAL AND PHOTOCOPY)

1. NEW APPLICATION – ADULT

Malinaw na kopya ng PSA BIRTH CERTIFICATE (dapat tama ang spelling at detalye ng aplikante)
Malinaw na kopya ng PSA MARRIAGE CERTIFICATE kung Kasal (dapat tama ang spelling at detalye ng aplikante mula sa PSA Birth Certificate at Marriage Certificate)
LOCAL CIVIL REGISTRAR COPY NG BIRTH CERTIFICATE mula sa Town/City Registrar kung ito malabo o hindi mabasa ang detalye sa PSA Birth Certificate
Isang VALID ID

MGA TINATANGGAP NA VALID ID

Philippine Identification (PhilID) / ePhilID
Social Security System (SSS) Card
Government Service Insurance System (GSIS) Card
Unified Multi-Purpose Identification (UMID) Card
Land Transportation Office (LTO) Driver’s License (Driver’s License cards expiring on April 24 until October 31 2023 and said expired cards perforated during the renewal process are accepted together with the temporary license in paper format).
Professional Regulatory Commission (PRC) ID
Overseas Workers Welfare Administration (OWWA) E-Card
Commission on Elections (COMELEC) Voter’s ID or Voter’s Certificate issued from COMELEC main office in Intramuros, Manila.
Philippine National Police (PNP) Permit to Carry Firearms Outside Residence
Senior Citizen ID
Airman License (issued August 2016 onwards)
Philippine Postal ID (issued November 2016 onwards)
Seafarer’s Record Book (SRB) or Seafarers Identity Document (SID) (*must be issued Feb 2020 onwards) issued by the Maritime Industry Authority (MARINA)

2. NEW APPLICATION – MINOR (KASAL ANG MAGULANG – MAGULANG ANG KASAMA MAG-APPLY)

Malinaw na kopya ng PSA BIRTH CERTIFICATE (tama ang spelling at detalye ng aplikante)
Malinaw na kopya ng PSA MARRIAGE CONTRACT ng MAGULANG (dapat tama ang spelling at detalye ng mga magulang)
Isang VALID ID ng Magulang (ang listahan ng ID ay nakasulat sa itaas)
Isang ID NG MINOR (7 years old and above)

School ID (if applicable) for the Current School Year
If school ID is not applicable, for minor - Certificate of Enrolment with photo of minor and dry seal of school
For Senior High/College applicants - School ID and Certificate of Registration

3. NEW APPLICATION – MINOR (HINDI KASAL ANG MAGULANG)

Malinaw na kopya ng PSA Birth Certificate (tama ang spelling at detalye ng aplikante)
Kung hindi Ina ang kasama sa pag-apply, Special Power of Attornery mula sa nanay ng minor.
Kung nasa ibang bansa ang nanay, Special Power of Attornery mula sa nanay ng minor na may tatak mula sa pinakamalapit na Philippine Embassy kung saan naroroon ang nanay ng bata.
Isang Valid ID ng Ina o Authorized Representative (ang listahan ng ID ay nakasulat sa itaas)
Isang ID ng Minor (7 years old and above)

School ID (if applicable) for the Current School Year
If school ID is not applicable, for minor - Certificate of Enrolment with photo of minor and dry seal of school
For Senior High/College applicants - School ID and Certificate of Registration

Paalala: May additional requirement para sa batang mag-tatravel na hindi magulang ang kasama, mangyari lamang po na tumawag kayo sa opisina ng Department of Foreign Affairs kung saan kayo mag-pa-file ng appointment para sa karagdagang requirements.

4. NEW APPLICATION – MINOR (KASAL ANG MAGULANG PERO PAREHONG NASA ABROAD)

Malinaw na kopya ng PSA Birth Certificate (tama ang spelling at detalye ng aplikante)
Malinaw na kopya ng PSA Marriage Contract ng Magulang (tama ang spelling at detalye ng mga magulang)
Special Power of Attornery mula sa mga magulang ng minor na may tatak mula sa pinakamalapit na Philippine Embassy kung saan naroroon ang nanay ng bata.
Isang valid ID ng Authorized Representative (ang listahan ng ID ay nakasulat sa itaas)
Isang ID ng Minor (7y/o and above)

School ID (if applicable) for the Current School Year
If school ID is not applicable, for minor - Certificate of Enrolment with photo of minor and dry seal of school or Certificate of Enrolment, signed by the school Principal (or any other authorized school representative), with dry seal of school stamped on the photo of the student who is the minor passport applicant.
For Senior High/College applicants - School ID and Certificate of Registration

5. RENEWAL – ADULT

Kung babae na magpapalit ng apelyido, PSA Marriage Contract & PSA Birth Certificate

6. RENEWAL – ADULT – LOST VALID PASSPORT

Affidavit of Loss in English
Police Report in English
