 How to Apply for an SSS UMID Card: Requirements and Steps

To apply for a Unified Multi-Purpose ID (UMID) card through the Social Security System (SSS) in the Philippines, follow these steps:

 

✅ Eligibility Requirements

You must meet the following criteria:

  • Be an SSS member with at least one posted contribution.
  • Have no existing UMID card issued by SSS, GSIS, or PhilHealth.
  • Have accurate and updated personal information in the SSS database.

 

📄 Required Documents

Prepare the following:

  1. Completed UMID Card Application Form

    Download it here: UMID Application Form (PDF)

  2. Valid Identification

    Present one primary ID with photo and signature, such as:

    • Passport
    • Driver’s License
    • SSS ID
    • Voter’s ID
    • NBI Clearance

    If you don't have a primary ID, present two secondary IDs, both bearing your name and signature, with at least one having your photo.

     

🗓️ Application Steps

  1. Schedule an Appointment

    Log in to your My.SSS account and set an appointment at your preferred SSS branch.

  2. Visit the SSS Branch

    On your appointment date, bring your completed application form and valid IDs.

  3. Biometric Data Capture

    At the branch, your photo, fingerprints, and signature will be captured.

  4. Acknowledgment Stub

    After completing the process, you'll receive a stub as proof of your application.

     

💳 UMID-ATM Pay Card Option

You can opt to have your UMID card function as an ATM card for receiving SSS benefits and loans. Indicate your preferred SSS-accredited bank on the application form. You'll claim the UMID-ATM card at the bank's branch and enroll it via the Disbursement Account Enrollment Module in your My.SSS account.

 

🕒 Processing Time

The UMID card is typically processed and delivered within 30 to 45 days. If you haven't received it within this period, check the status through your My.SSS account or contact the SSS branch where you applied.

 

💰 Fees

  • First-time Application: Free of charge.
  • Replacement (lost, damaged, or updated information): ₱200 fee. 


"Please note: As of now, the SSS is not issuing UMID cards. Instead, they are providing a UMID Certification, which serves as a temporary replacement while the UMID card is unavailable. Please wait for official updates from SSS regarding the resumption of card issuance."
