Affiant 1:

A disinterested person (not a relative and with no vested interest) who has personally known Rizza Laurel for a long time. This person serves as the first witness confirming that the true and correct name of the individual is Rizza Laurel, and that she has no middle name. The affiant may be a neighbor, community acquaintance, or a barangay official.

Affiant 2:
The second disinterested person who is also not related to Rizza Laurel but personally knows her. This person confirms that the inclusion of "NMN" in official records is a clerical error and does not reflect her true name as shown in her birth certificate.
